Our office has preferred providers for Delta Dental, HealthPartners,
Premier, Formula, Ameritas, UCare for Seniors, Singular Dental and
CAPS. Whether we are in network or out of network providers for
your insurance plan, we will gladly process your claims and assist
you in receiving the maximum benefits from your plan. However, an
insurance policy is a contract between the insurance company and
the insured, and most policies do not cover 100% of treatment costs.
As your dental care provider, our relationship is with you, the
patient, not your insurance carrier and you are ultimately responsible
for payment. If payment is not received from your insurance carrier
within 60 days of filing the claim, you may be required to make
other payment arrangements with us.
